New Patients

Capital Foot and Ankle is currently accepting new patients.

Most insurance plans are accepted.  Please click on the link below to see a list of current insurances we accept. If you do not see your insurance listed, you may call our billing department. Insurance verification is highly recommended prior to your appointment.

NEW PATIENT FORMS

Click on the link below to access our new patient forms. Please fill out the forms and sign them prior to your first appointment. Completing the new patient forms at minimum 24 hours prior to your appointment will save you time at your initial visit. 

SCHEDULING AND CANCELLATIONS

Please call one of our offices to schedule an appointment. If you are unable to make your appointment, please notify us 24hrs in advance.

** Please arrive 15 minutes prior to your scheduled appointment if you are scheduled for Sacramento or Folsom.

**Please arrive at least 30 minutes prior to your scheduled.appointment if you are scheduled for Roseville. If you are unable to find parking, the parking garage across from our building(Building 5) offers complimentary valet parking.

  • Photo ID

  • Insurance card

  • Primary care physician info

  • List of allergies and current medications

  • Recent medical records or xrays

  • New Patient forms if unable to fill out online
